Who surveys you
Skilled nursing. CMS life safety surveys, run in Pennsylvania by the Department of Health's Division of Safety Inspection and in New Jersey by the Department of Health. Pennsylvania nursing homes are surveyed every 9 to 15 months.
Personal care homes and assisted living in Pennsylvania are licensed by the Department of Human Services under 55 Pa. Code Chapters 2600 and 2800.
CCRCs often face both, on one campus.
If your community is accredited by The Joint Commission, note that some non-hospital programs, such as assisted living, still score life safety under the older LS.02 numbers.

What your report gives you
One page up front shows doors inspected, pass rate, failures by type and the cost to fix. Behind it, every door, damper and barrier has its own line with location, result, code reference and photo. Repairs are priced and sorted by risk, so you fix what a surveyor will see first. Hand it to the state surveyor as your written record. See a sample inspection report.
